New GLP-1 pill achieves up to 12% weight loss in 36 weeks

Oral obesity treatment shows promising results in clinical trial.

1Novel treatment approach

A phase II clinical trial published in Nature Medicine found that aleniglpron, a new GLP-1 pill, led to up to 12% body weight loss in obese or overweight adults over 36 weeks. The study followed 230 participants (average age 50) across 38 US medical centers. Unlike current injectable GLP-1 drugs, this small-molecule formulation can be taken orally.

2How GLP-1 drugs work

GLP-1 medications mimic the natural GLP-1 hormone, stimulating insulin secretion, reducing appetite and increasing satiety to support weight loss. However, existing peptide-based drugs require injections, have strict storage requirements (refrigeration) and are costly to manufacture at scale. The small-molecule version could overcome these limitations.

3Clinical trial details

In the randomized, double-blind, placebo-controlled study, participants were assigned to 45 mg, 90 mg or 120 mg dose groups. The pill was taken daily with doses increased every 4 weeks. By week 36, average weight loss was 9% (45 mg), 10.7% (90 mg) and 12.1% (120 mg), compared to just 0.5% in the placebo group.

4Safety profile

Gastrointestinal side effects were generally mild to moderate and decreased in frequency as the trial progressed. While 10.4% of participants discontinued treatment, there were no cases of drug-induced liver injury. Dr. Robert Kushner from Northwestern University noted no safety concerns and plans to slow dose escalation in phase III to improve tolerability.

5Comparison with existing treatments

Current GLP-1 drugs like semaglutide (Ozempic/Wegovy) require injections and have limited accessibility. Aleniglipron is chemically synthesized like other small-molecule drugs (e.g., aspirin) and can be taken with or without food. These properties offer advantages for combination therapies and large-scale manufacturing.

6Phase III preparations

Researchers confirmed they've identified effective doses and plan to slow dose escalation in phase III trials to enhance tolerability. Small-molecule GLP-1 drugs could represent a significant advancement by making obesity treatment more accessible and affordable through easier production and distribution.

7Patient compliance benefits

The oral formulation is expected to improve patient adherence, particularly for those with needle phobia or difficulty administering injections. Not requiring cold chain storage also simplifies distribution and storage, potentially expanding access to treatment in various healthcare settings.

8Potential game-changer

These findings mark significant progress in obesity treatment. Oral GLP-1 therapy could reach millions more patients. If phase III trials succeed, aleniglipron may become a quality-of-life improving option for obese individuals at risk of diabetes and cardiovascular disease, addressing a major global health challenge.
